Who is a carer?
A carer is a person of any age who has unpaid caring responsibilities for someone with a physical disability, physical illness or mental ill health. The person needing care may be a relative, partner, friend or neighbour who would not be able to manage without assistance.
What we can do to help

We are aiming to compile a comprehensive register of our patients who are carers who have agreed to be on the register. This will enable us to send information on available support and arrange referral to Social Services and other voluntary service agencies if wanted.

If all of our staff are able to recognise you as a carer we can ensure you receive appropriate consideration and support.

We can offer a flexible approach to consultations and to obtaining prescriptions if leaving the dependent person is difficult.

How can you register as a carer?
Forms are available in surgery. You can request more information by e mail or speak to any staff member.
Margaret Jones, our Health Care assistant, is our Practice Carers Co-ordinator. You can ask to speak to her by telephone or make an appointment for a chat in one of her clinics.
What is a Carer's assessment ?

A Carer's assessment is your opportunity to tell Social Services about the thing that could make caring easier for you. It is an assessment of your needs, not those of the person you are caring for.
Every carer is entitled to an assessment. When you register with us as a carer we will offer you the chance of a referral. Useful link with detailed information here

How to find out more information

We have a dedicated Carers notice board in the waiting room with a lot of information and news of local events.

The British Red Cross Carers' Fieldworker Project is active in Ceredigion. When you register as a carer we will give you details of the practical help, advice, support and activities available. You can contact Sally Bates on 01239 615945 for more information.

Ceredigion Carers Emergency Card. This can be used as instant identification if a carer is taken ill suddenly or has an accident. The British Red Cross hold the details so help can be quickly summoned to look after the dependent person.

Carers Wales is an organisation working for a better deal for Carers in Wales. They have a web site with useful details of financial and practical help and events.
ACE National Action for Carers and Employment is working to support the inclusion of carers in training and work.
Crossroads is a voluntary organisation that can provide respite care when you need a break. Local contact details are available on their web site or from surgery.
PAVS Pembrokeshire association of voluntary services develops services for carers in Pembrokeshire. To receive a regular newsletter or to be invited to future Carers events contact Sally Davies 01437 771196
CareAware is a helpful website for advice about funding care for an older person.
Ceredigion_County_Council The health and social care section has comprehensive information for carers locally. The interesting Carers newsletter, JIGSAW, can be downloaded from this site.
The Princess Royal trust for Carers - the largest provider of comprehensive carers support in the UK. The interactive website has very useful information and a discussion board for carers.
